By using this website you agree to these terms. If you do not agree, please do not use the site. Hormone Balance Centers (“HBC”, “we”, “us”) may update these terms, and the date above shows the latest version.

Not medical advice

Content on this website, including articles, guides and pricing information, is general education. It is not medical advice, a diagnosis or a treatment recommendation for you, and it does not replace an evaluation by a licensed clinician. Do not start, stop or change any medication based on what you read here.

Medical emergencies

This website, our phone line and our text line are not for emergencies. If you think you are having a medical emergency, call 911 or go to the nearest emergency room.

No patient relationship from using the site

Visiting this website, calling or texting us does not by itself create a provider-patient relationship. That relationship begins only after you complete intake and are accepted for care by one of our clinicians.

Telehealth care

• We provide care by telehealth to eligible adults located in Utah and Idaho at the time of each visit.
• Your clinician decides which laboratory tests are appropriate and whether treatment is medically appropriate. Not every patient is a candidate for treatment, and we prescribe only when it is medically appropriate.
• Telehealth has limits. Your clinician may recommend in-person evaluation or care elsewhere when that is safer or more appropriate.

Fees and payment

HBC is a self-pay practice and does not bill insurance directly. Current fees are published on our pricing pages and confirmed before care begins. Medication, laboratory testing and pharmacy services may be provided and billed by separate businesses under their own terms.

Third-party services and links

The patient portal, pharmacies, laboratories and other services we link to are operated by separate companies. We are not responsible for their content, availability or practices.

Using this website

Please do not misuse the site: do not attempt to interfere with its operation, gain unauthorized access, or copy its content for commercial use. Website content, logos and design belong to HBC or its licensors.

Disclaimers

We work to keep information accurate and current, but the website is provided “as is” without warranties of any kind. Individual results from any treatment vary.

Limitation of liability

To the fullest extent allowed by law, HBC is not liable for indirect, incidental or consequential damages arising from your use of this website. Nothing in these terms limits liability that cannot be limited under applicable law.

Governing law

These terms are governed by the laws of the State of Utah, without regard to its conflict-of-law rules.
